For the most part, they just ignore it.

The FTC has put a lot of effort into this, but the advent of cheap VoIP
means that you can now annoy a lot of people for very little money and
it is easy to disappear and reappear somewhere else.

ObCrypto: another effect of VoIP is that you can lie with impunity
about the number you're calling from.  The FCC is working on digital
signatures for VoIP calls but it's a long slog.  The technology is
simple enough in concept but when you look at the mountain of kludges
that is the modern phone system, it's a lot of work to get it
deployed.
