This has been researched extensively in the context
of mobile devices. That isn't a specific answer to
your question, but it tells you where to look, e.g.

One specific thing that is not particularly lightweight,
but is guaranteed to work, is to have a box with a
static IP address, which you can consider a proxy or
simply a router. It then forwards stuff to the
endpoint ... possibly using a succession of IPsec
tunnels.
